How much of that risk is directly caused by homocysteine itself, rather than reflecting other things going on in the body, is still debated, and the strength of the association appears to vary between population groups.
It's not a replacement for a standard lipid panel. It's a different lens on the same risk. What is homocysteine, and why does a heart test measure it?
Homocysteine is a by-product of breaking down methionine, an amino acid that comes from protein in your diet. In a well-functioning body, enzymes recycle homocysteine almost as fast as it's produced, converting it back into other useful compounds. That recycling process depends on adequate vitamin B12, folate and vitamin B6.
When the recycling process slows down, whether from a nutrient shortfall, a genetic variant, or another contributing factor, homocysteine accumulates in the blood. Research has linked persistently raised homocysteine to blood vessel strain and impaired vessel repair, which is why it's grouped with markers such as CRP as part of a broader inflammation and cardiovascular risk picture, rather than read alone.
What does a raised homocysteine result usually mean?
A raised result doesn't diagnose heart disease, and it should always be read alongside your other cardiovascular markers and history, ideally with your GP or practitioner. What it does flag is one of a few underlying patterns worth investigating:
- A shortfall in vitamin B12, folate or B6, the nutrients your body needs to clear homocysteine efficiently
- A genetic variant affecting the same recycling pathway, most commonly in the MTHFR gene
- A contributing lifestyle or health factor, covered below
Because several of these causes are straightforward to address once identified, homocysteine is one of the more actionable markers on a heart panel.
What causes elevated homocysteine?
Vitamin B12, folate and B6. These three B vitamins drive the enzyme pathways that clear homocysteine from the blood. A shortfall in any one of them, whether from diet, absorption issues or increased needs, is one of the most common and most fixable causes of a raised result.
MTHFR gene variants. The MTHFR gene provides instructions for an enzyme central to folate metabolism. Common variants such as C677T and A1298C can reduce that enzyme's efficiency, which may make it harder for the body to process folate and, in turn, clear homocysteine. MTHFR variants are common in the general population, and carrying one is a marker relevant to homocysteine handling rather than a diagnosis in itself. If your homocysteine result stays raised despite adequate B vitamin intake, an MTHFR blood test is one way to explore whether a variant is a contributing factor, discussed with your practitioner. The clinical significance of MTHFR variants beyond this pathway is still debated, so we'd rather under-claim here than over-promise.
Other contributing factors. Smoking, high coffee or alcohol intake, some medications including proton pump inhibitors, diabetes, reduced kidney function and underactive thyroid function have all been associated with higher homocysteine levels in research.
Homocysteine reference ranges
Laboratories vary slightly in exactly where they draw the line, so always check the specific range printed on your own report. As a general guide:

What can help bring a raised level down
Where a raised result is linked to a B vitamin shortfall, addressing that shortfall (through diet and, where appropriate, supplementation guided by your practitioner) is typically the first step. Where an MTHFR variant is a contributing factor, your practitioner may suggest a specific form of folate rather than a generic one. Reducing smoking and heavy alcohol or coffee intake, and managing conditions such as diabetes or thyroid dysfunction, are the other levers worth discussing.
It's worth being clear about what this does and doesn't achieve. Correcting a B vitamin shortfall or an MTHFR-related folate issue reliably brings the homocysteine number itself down. Large clinical trials of folic acid and B vitamin supplementation have not shown this translates into fewer heart attacks or lower cardiovascular deaths, though some analyses point to a modest reduction in stroke risk. These measures are best understood as correcting an underlying nutrient or genetic factor, rather than as a proven heart-protective treatment in their own right.
