What is a sperm DNA fragmentation test?
Every sperm carries a tightly packed copy of half a person’s DNA. Fragmentation means breaks in those DNA strands. A standard semen analysis counts sperm and assesses how they move and how they are shaped — it cannot see inside them. A man can have a completely normal semen analysis and a high proportion of sperm carrying damaged DNA.
The result is reported as a DNA Fragmentation Index, or DFI: the percentage of sperm in the sample showing DNA damage. A higher percentage has been associated in research with a lower chance of natural conception, lower success with intrauterine insemination, and a higher rate of miscarriage. The association is a statistical one across groups of men, not a prediction about any individual.
This test includes the full semen analysis as well, so you get both the standard measurements and the DFI from a single sample.
Who this test is for?
This isn't a first test. If you haven't had a semen analysis yet, start with the Home Semen Test — it's cheaper, it answers the more common questions, and the result tells you whether this one is worth doing.
DNA fragmentation testing is usually considered when:
- You've had recurrent miscarriages
- You're about to start IUI (intrauterine insemination)
- IVF or ICSI cycles haven't worked
- Nothing has been found to explain difficulty conceiving, and the semen analysis looked normal
- You have a varicocoele
- You smoke, carry extra weight, work around heat, or have recently been seriously unwell
The evidence behind this test is still developing, so it's one to talk through with your doctor rather than order on a hunch.
