Please only collect your sample on a Sunday or Monday, and make sure it is returned on Monday.

Before collecting your DNA sample first thing in the morning, make sure you have fasted overnight. This means no food, drink, oral care, smoking, or chewing gum. Please also wash your hands thoroughly before handling the collection kit. This helps reduce the risk of sample contamination and ensures the most accurate results from your test.

1. Fill Out and Sign Your Request Form

This is a vital part of your DNA test submission. Please follow these steps carefully:

  • Each test kit includes a request form that is automatically pre-filled with the details of the person who placed the order.
  • If you are ordering for someone else (for example, a child or spouse), you must cross out the pre-filled information and clearly write the correct patient details.
  • The form must be signed by the individual providing the sample, or by a parent or legal guardian if the sample is for a child.
  • Each patient has a unique patient number, located under the QR code in the top right corner of the request form. This number is how we track your sample and match it to your account.

  • You must also clearly label the DNA swab tube with your unique patient number (under 'Evidence'), full name (under 'Collected by'), and date of birth (under 'Date').

If you update or change any patient information, please contact info@i-screen.co.nz.

A separate i-screen account with a unique email address is required for each patient, including children. If you only have one email address, you can use the '+' trick to create variations. For example, if your email is parent@example.com, you can register your child as parent+childname@example.com. Emails with '+' still go to your original inbox but are treated as unique by our system.

2. Sample Collection Instructions

  • Make sure you have fasted overnight. This means no food, drink, oral care, smoking, or chewing gum. Please also wash your hands thoroughly before handling the collection kit.
  • Make sure the swab and the inside of the collection tube do not come into contact with any surfaces.
  • Collect your sample by firmly rubbing the swab against the inside of both cheeks for a total of 60 seconds.

  • Place the swab back into the tube and seal it securely.

3. Packaging Your Sample for Return

  • Place the labelled DNA tube inside the biohazard bag.

  • Insert the completed and signed request form into the outside pocket of the biohazard bag (do not place it inside with the tube). If you have ordered multiple tests, please make sure only one swab and its accompanying paperwork is placed in each biohazard bag.

  • Then place the entire biohazard bag into the provided return mailer.

Return Instructions (New Zealand)

  1. Use the prepaid envelope. Return your sample using the labelled prepaid bubble mailer provided in your kit.
  2. Send it off. You can return your DNA sample by dropping it at any NZ Post Shop.
  3. Results. You can expect your results to appear in your online dashboard within 3 to 4 weeks of the sample being sent back to the lab.
